Even though 3-D Imaging, P.C. no longer offers 3-D ultrasound services to the general public, the following terms and conditions still apply to customers who used the services of 3-D Imaging, P.C. prior to its discontinuance of this service.

Terms and conditions: In purchasing the services offered by 3-D Imaging, P.C., customer is subject to all of the terms and conditions stated herein. This service is not offered as a medical treatment or diagnosis. To use this service, the customer must fill out and sign a contract. Customer must be under the care of a physician and disclose the name of her physician in the contract. During the ultrasound session(s), should suspected problems be observed, it is the sole responsibility of the customer to make her physician aware of any suspected problems. Since the ultrasound sessions are purely for non-medical purposes, under no conditions will 3-D Imaging, P.C.; its affiliates; or its staff be held liable or accountable for the consequences of any suspected conditions or failure to diagnose, treat, or inform about said suspected conditions.

In documenting the session, the best efforts of the staff will be made to obtain meaningful pictures or video of the fetus showing gender and other body features. However, due to circumstances beyond the control of 3-D Imaging, P.C. such as the position of the fetus(es), the placenta, or internal organs, no guarantees can be made as to picture or video content. 3-D Imaging, P.C. shall attempt to choose material which the customer will like but should staff and the customer be unable to agree upon which still images to print or save to floppy disk, or which live action and rendered sequences to videotape, it is the sole prerogative of 3-D Imaging, P.C. to choose material it considers appropriate.

Under no circumstances shall 3-D Imaging, P.C. be held liable for any injury or damages that may occur to the customer (including the customer's fetus(es) as a result of undergoing any ultrasound session(s).

Prices and contents of a session are subject to change without notice. From time-to-time, special promotional offerings may be made available. Such offerings may be discontinued at any time without notice. Payment may be made with cash or credit card only. No personal checks accepted.
